Project-to-Operations HSE Gate Checklist

Five control gates for moving a project into commissioning and operations without transferring unmanaged risk.

Five transition gates

Each gate needs named acceptance criteria, objective evidence and an accountable owner representative.

  • Gate 1: design risk and operational intent
  • Gate 2: construction completion and defect control
  • Gate 3: commissioning isolation and energization readiness
  • Gate 4: people, procedures, emergency and maintenance readiness
  • Gate 5: operational acceptance and residual-risk ownership

No-go conditions

Do not accept schedule pressure as evidence of readiness. Open fatal-risk controls, unclear ownership, incomplete emergency capability or unverified energization boundaries require escalation before transition.
