Begin with the conditions
Ask what made the action sensible to the person at the time. Examine knowledge, training, workload, supervision, equipment, norms and competing demands.
Test the decision fairly
Compare the action with what peers understood and typically did in the same conditions. Distinguish a predictable system outcome from a conscious and unjustifiable choice.
Match the response
Learning, coaching, system correction and accountability serve different purposes. Apply the response that addresses the cause and protects future reporting trust.
