Ten questions for leaders

Score each question 0 for no evidence, 1 for partial evidence and 2 for consistent field evidence.

• Can leaders name the current highest-energy exposures?
• Are critical controls defined with clear performance requirements?
• Is verification based on field evidence rather than paperwork alone?
• Can workers explain when to stop and who will support them?
• Are failed controls escalated at the speed of the risk?
• Do investigations examine control and organizational conditions?
• Are contractor exposures visible in the same risk picture?
• Are leading indicators connected to decisions and actions?
• Does the system detect behavioral drift before an event?
• Can the executive team explain what changed this month?

Interpret the score

A low score signals immediate gaps. A high score still needs field validation. Use the result to choose where independent assurance will add the greatest value.
