The field question
A control is not verified because a document says it exists. Verification establishes that the control is present, effective and understood at the point of exposure.
Use three levels of evidence
Ask for evidence that is specific to today’s work and conditions.
- Presence: Is the control physically or procedurally in place?
- Performance: Is it working to the required standard?
- Resilience: What could defeat it during change, pressure or abnormal conditions?
Close the loop
Record failed or degraded controls, identify the exposure created, assign authority for correction and verify restoration before work continues.
